Insert lines in the verticle axis of bar chart

Hi Experts,

Please help us here-

We have requirement to prepare a chart  as shown below. The lines which separate the region as Poor, Medium and Good will be taken from table. Can you please suggest how to bring these lines on the bar chart by retrieving these data from the table.

We have configured the Poor, Medium and Good values in the table. So the lines should be drawn dynamically based on the configured values in the table.
