How to make a two row group header that repeats as needed?

Using SSRS 2008 R2.  Really struggling to get a group header with two rows that will repeat with the group's detail.  There are four fields included in the dataset.  I would like the first 3 fields in the top row of the group header and the last one in the second row.  The dataset can be grouped on the first 3 fields but I include the 4th field in the group because I need it to print on each page that the report's subreport data appears.

***Field4***

*****************************************

Subreport

*****************************************

If anyone has time to help guide me through this I would appreciate it. 

I'm trying to follow this example -

However, it appears to only be grouped by a single field.  Maybe I don't have my group setup correctly since I'm using multiple fields?  I'm trying to use a "table" but maybe I should use a "matrix".
